Application for Citizenship Status Under Article 15(2) of the Federal Constitution – Child of a Citizen Under the Age of 21

Registration application sebagai citizen Malaysia by seseorang that aged dibawah twenty-one years, jika sekurang-kurangnya salah seorang from ibu bapanya (or pada masa deathnya) seorang citizen.

1. The applicant and the person being registered must be present before the Citizenship Registrar when submitting the application.
2. The person being registered must be under the age of 21 on the date the application is submitted.
3. Salah seorang dari ibu bapanya is seorang citizen Malaysia.
4. The application must be submitted by mother or fathernya or penjaga.
5. Complete Form B and enclose copies of the required supporting documents.
6. Complete Appendix A.15(2) namely Surat Akuan Consent bersama application citizenship Article 15(2) Federal Constitution that submitted di Consular Office abroad.

The applicant and the person being registered are required to be present at the Citizenship Division counter NRD Headquarters, Putrajaya or State NRD Headquarters/NRD Cawangan and bring the original documents together with copies.

1. DOCUMENTS OF THE PERSON BEING REGISTERED (ORIGINAL AND COPY)
   • Identity card (if any).
   • Birth Certificate or Extract Birth Certificate terbaru or Certificate Adopted Child (Translation of the birth certificate in Malay or English by the Court/Consular Office/public translator if the child was born abroad).
   • Permit Masuk or pasport or lain-lain documents perjalanan (if any)
   • Tiga (3) keping photograph terbaru berpassport size berlatar belakangkan warna biru.
   • Documents bukti persekolahan (jika bersekolah)
     
2. APPLICANT OR GUARDIAN DOCUMENTS (ORIGINAL AND COPY)
   • Identity Card
     
3. DOCUMENTS IBU BAPA & ADIK BERADIK (ORIGINAL AND COPY)
   • Birth Certificate or Extract Certificate birth terbaru
   • Identity Card
   • Certificate Citizen (if any)
   • Documents MarriageIdentity card (if any)
     
     Nota: Semua copy fotostat documents-documents sokongan must dibuat on A4-sized paper

1. RM10.00 fees registration application.
2. RM100.00 fees pengeluaran Certificate Citizen (jika is approved).

Semua application urusan citizenship must menggunakan form original . Form-form bby didapatkan from NRD Headquarters, Putrajaya or mana-mana State NRD Headquarters/Cawangan. For application registration birth abroad and renunciation citizenship, form bby juga didapatkan dari Consular Office abroad.

Application Citizen Bawah Article 15A and 15(2) Federal Constitution FORM B (obtain at the NRD counter) has been completed in full.
