Application for Citizenship Status Under Article 19 of the Federal Constitution – Aged 21 Years or Above

to obtain a certificate as a Malaysian citizen by naturalisation.

• Applicant must be present before the Registrar Citizen semasa mengemukakan application.
• Applicant must aged 21 years and above pada date application dikemukakan.
• The applicant has resided in the Federation for a total period of not less than 10 years in the preceding 12 years, including the 12 months immediately before the date the application is submitted.
• The applicant intends to reside permanently in the Federation.
• Applicant berkelakuan baik.
• Applicant mempunyai pengetahuan Bahasa Malay that memadai.
• Applicant must ditaja by dua orang penyokong bertaraf citizen aged 21 years and above, tiada talian persaudaraan, bukan orang that diupah or lawyerbela or lawyercara with applicant.
• Complete Form C and enclose copies of the required supporting documents.

Applicant is required to be present ke Counter Bahagian Citizenship NRD Headquarters, Putrajaya or State NRD Headquarters /Cawangan and bring the original documents together with copies.

   • Identity card.
   • Permit Masuk or Extract Butir-Butir dari Register Identity Card.
   • Pasport or lain-lain documents perjalanan (if any)
   • Birth Certificate or Extract Certificate birth terbaru or Certificate Adopted Child (if any).
   • Tiga (3) keping photograph terbaru berpassport size berlatar belakangkan warna biru.

   • Identity Card
   • Birth Certificate or Extract Certificate birth terbaru (if any).
   • Certificate citizen (if any).
     
3. DOCUMENTS SUAMI, ISTERI & ANAK- ANAK (COPY)
   • Identity Card (if any).
   • Birth Certificate or Extract Certificate birth terbaru (if any).

4. PARENTS AND SIBLINGS DOCUMENTS (COPY)
   • Identity Card (if any)
     
     Nota: Semua copy fotostat documents-documents sokongan must dibuat on A4-sized paper

1. RM10.00 fees registration application.
2. RM200.00 fees pengeluaran Certificate Citizen for that berada in the Federation pada Merdeka Day, 31 Ogos 1957 (jika is approved).
3. RM300.00 fees pengeluaran Certificate Citizen for that tidak berada in the Federation pada Merdeka Day, 31 Ogos 1957 (jika is approved).

Semua application urusan citizenship must menggunakan form original . Form-form bby didapatkan from NRD Headquarters, Putrajaya or mana-mana State NRD Headquarters/Cawangan. For application registration birth abroad and renunciation citizenship, form bby juga didapatkan dari Consular Office abroad.
