Application for Extract or Certified True Copy from the Citizenship Register
Those wishing to obtain an extract or a certified copy of an Application Form, Certificate, Notice, Order, or Entry in the Register issued under Part III of the Constitution or under the provisions of the Federation of Malaya Agreement, 1948, may do so by submitting a written application using the prescribed form.
Complete Form N and include A4-sized copies of the required documents to be brought along.
Applications may be submitted at any Citizenship Division counter at the NRD Headquarters in Putrajaya, or at any State NRD Headquarters or branch office.
- Details for the issuance of the extract (such as Certificate No., Subject, and place and date of issuance).
- Applicant’s identity card number (if the applicant’s name is listed on the parent’s Certificate of Citizenship, attach the birth certificate).
- If the applicant is the child of the certificate holder and the parent (the certificate holder) is deceased, attach the parent’s death certificate.
- Clearly state the reasons for requesting the extract in the application form.
- Applications may be submitted by:
- The document owner; or
- The document owner’s parent(s); or
- The document owner’s spouse; or
- The document owner’s child; or
- The nearest next-of-kin (grandparents only); or
- A lawyer holding a letter of authorization. A lawyer submitting an application on behalf of a client must provide full details—including their name and identity card number—and affix the official law firm stamp to the Application Form.
- RM2.00 application registration fee.
- RM5.00 document issuance fee.
All applications regarding citizenship matters must be submitted using the original forms. These forms can be obtained from the NRD (JPN) Headquarters in Putrajaya or any State NRD Headquarters or branch office. For applications concerning the registration of overseas births and the renunciation of citizenship, forms may also be obtained from consular offices abroad.
