Application for Extract or Certified True Copy from the Citizenship Register
For they that wish to obtain extract or copy that sah ke atas Form Application, Certificate, Notis, Perintah, Akuan In Register that diberi under Part III of the Constitution or under the provisions of the Federation of Malaya Agreement, 1948, may be made in writing using the prescribed form.
Complete Form N and enclose A4-sized copies of the required supporting documents.
Application bby diterima di mana-mana counter Bahagian Citizenship NRD Headquarters, Putrajaya /State NRD Headquarters/Cawangan.
- Butir-butir information to pengeluaran extract (such as No. Certificate, Article, tempat and date dikeluarkan).
- No. Identity card applicant (jika nama applicant tercatat in Certificate Citizen ibu/bapa, kembarkan birth certificate).
- Jika the applicant is the child of the certificate holder requested and the parent who is the certificate holder has died; enclose the death certificate of the parent.
- Berikan sebab-sebab memohon extract with jelas in form application.
- Applicanton bby dibuat by:
- Pemilik documents or
- Ibu/bapa pemilik documents or
- Suami/isteri pemilik documents or
- Anak to pemilik documents or
- Next of kin terdekat (datuk/nenek sahaja)
- Peguam that diberikan surat wakil sahaja. A lawyer submitting an application on behalf of a client must provide complete information, including the lawyer name and identity card number, and affix the official legal firm stamp on the application form.

Semua application urusan citizenship must menggunakan form original . Form-form bby didapatkan from NRD Headquarters, Putrajaya or mana-mana State NRD Headquarters / Cawangan. For application registration birth abroad and renunciation citizenship, form bby juga didapatkan dari Consular Office abroad.
