Application for Renunciation of Citizenship Status Under Article 23 of the Federal Constitution
For persons aged twenty-one years or above, of sound mind, and who have become or will become foreign citizens by renouncing Malaysian citizenship. For married women, the age factor is not taken into account.
- Applicant must aged 21 years and above.
- Sempurna akal.
- Has acquired foreign citizenship status or has certification from the relevant country that he/she will become a foreign citizen.
- Complete Form K in two copies and enclose copies of the required supporting documents.
Applicant is required to be present sendiri di mana-mana counter Bahagian Citizenship NRD or di Malaysian Consular Office abroad with mecompletei 2 keping Form K (original).
- Copy pasport Negara asing
- Copy certificate citizen Negara asing or Copy Surat Pengesahan that applicant akan menjadi Citizen asing
- Certificate birth or Extract of birth certificate terbaru for birth di Malaysia or Form W for birth di Abroad (if any)
- Certificate Citizen (if any)
- Identity card (if any)
- Dua (2) keping photograph applicant berpassport size, berlatar belakang biru
- Cap ibu jari kiri and kanan applicant
- Form K must ditandatangani and disahkan dihadapan Majistret/Jaksa Pendamai/Peguamcara/Notari Awam or Pesuruhjaya Sumpah
- Pasport Malaysia or lain-lain documents perjalanan
- Documents marriage (jika perempuan that aged under 21 years old)
Nota: Semua copy fotostat documents-documents sokongan must dibuat on A4-sized paper
RM10.00 fees registration application.
Semua application urusan citizenship must menggunakan form original . Form-form bby didapatkan from NRD Headquarters, Putrajaya or mana-mana State NRD Headquarters/Cawangan. For application registration birth abroad and renunciation citizenship, form bby juga didapatkan dari Consular Office abroad.
