Application bby diterima di mana-mana counter Bahagian Citizenship NRD Headquarters, Putrajaya/NRD State/Cawangan or through pos.

1. Identity card or ambil information applicant tertentu pada identity card
2. Certificate Citizenship / Status Pengesahan Certificate Citizenship / Form W / Form K (Original)
3. Documents that disokong sebagai bukti amendment
4. Tiga (3) Photograph applicant berpassport size with latar belakang biru
5. Surat Beranak or extract Birth Certificate terbaru (if any)
6. Marriage document (if any)
7. Only copies of documents need to be sent for applications made by post
8. Applicant must:
   – Pemilik documents or;
   – Ibu bapa pemilik documents or;
   – Pasangan pemilik documents or;
   – Anak pemilik documents or;
   – Keluarga sebelah (datuk / nenek sahaja)
   
      Nota: Semua copy fotostat documents-documents sokongan must dibuat on A4-sized paper

1. RM10.00 fees registration application
2. RM10.00 fees pengeluaran documents

Semua application urusan citizenship must menggunakan form original . Form-form bby didapatkan from NRD Headquarters, Putrajaya or mana-mana State NRD Headquarters/Cawangan.
