Application for Search in the Death Register (Section 31) (Peninsular)

Application search is application to mendapatkan keputusan search mengenai death register with fi that designated. Search register in death bby dibuat di NRD Headquarters, Putrajaya and NRD Ibu State sahaja.

Application for an extract of death certificate may be made by the following individuals:

1. Mother or father the deceased ;
2. Husband or wife the deceased ;
3. Biological child or adopted child that sah for the deceased;
4. Biological siblings the deceased;
5. Agensi kerajaan;
6. Statutory Body;
7. Legal Firm.

KATEGORI 1: INDIVIDUALS IN CATEGORIES 1 TO 4

1. Form NRD.LM13 that complete;
2. Identity Card or identification document applicant (original and copy);
3. Copy death certificate the deceased (if ada).
4. marriage certificate suami and isteri (original and copy)

KATEGORI 2: INDIVIDUALS IN CATEGORY 5

1. Form NRD.LM13 that complete;
2. Official Letter Office Legal Firm to memohon search and extract in register;
3. Appointment letter from client (applicant);
4. Identity Card or identification document client (applicant) (original and copy);
5. Authorization letter to menguruskan application (if applicable).

KATEGORI 3: INDIVIDUALS IN CATEGORY 6 DAN 7

1. Form NRD.LM13 that complete;
2. Official Letter Government Agency or Statutory Body to memohon search or extract in register;
3. Authorization letter applicant to menguruskan application (if applicable).

KATEGORI 4: SELAIN DARIPADA INDIVIDUALS IN CATEGORIES 1 TO 4

1. Form NRD.LM13 that complete;
2. Documents pengenalan diri or documents perjalanan that sah (jika non-citizen).
3. Official Letter to memohon search in register.
4. Statutory Declaration (Jika berkaitan).
5. Salasilah/Susur galur keluarga (Jika berkaitan).
6. Authorization letter dari next of kin that layak bersama copy identity card next of kin (Jika berkaitan).
7. Sebarang documents or information that bby dijadikan asas membuat search in register birth seperti Extract Register Persekolahan and Birth Certificate siblings that lain (for application search birth).

Application Fee RM10.00

NRD.LM13 – Application Search/Extract (obtain at the NRD counter) has been completed in full.
