Application to Correct Information Under Section 25(3) in the Death Certificate for Correction of Factual Error (Sarawak)

Application to membetulkan kesilapan or memasukkan fakta di mana information that diberi informant semasa registration death tidak betul or tidak diperbyi.

Application correction information section 25(3) may be made by the following individuals:

1. Biological mother or father the deceased;
2. Husband or wife the deceased;
3. Biological siblings the deceased;
4. Biological child or adopted child that sah to the deceased;

1. Form XI and Form NRD.LM21 that complete;
2. Surat Statutory Declaration SA/81;
3. Death Certificate the deceased ( original and copy );
4. Identity Card or identification document applicant (original and copy);
5. Pasport or documents insurans or buku bank the deceased if the deceased mempunyai pasport or akaun bank or insurans (original and copy );
6. Certificate Nikah or marriage document for application that dibuat by husband or wife the deceased;
7. Documents-documents lain that bby menyokong application seperti Certificate or Kad Memeluk Agama Islam mother or father the deceased, Birth Certificate or Death Certificate mother or father the deceased and surat approval penukaran nama from Bahagian Identity Card, NRD;

1. Fees registration application RM2.00;
2. Fees extract Birth Certificate RM10.00.
