Application to Correct Information Under Section 27(3) in the Death Certificate for Correction of Factual Error (Peninsular)

Application to membetulkan kesilapan or memasukkan fakta di mana information that diberi informant semasa registration death tidak betul or tidak diperbyi.

Application for death registration may be made by the following individuals:

1. Biological mother or father the deceased;
2. Husband or wife the deceased;
3. Biological siblings the deceased;
4. Biological child or adopted child that sah to the deceased;

1. Form NRD.LM21 and NRD.LM22 that complete;
2. Death Certificate the deceased (original and copy);
3. Identity Card or identification document applicant (original and copy);
4. Pasport or documents insurans or buku bank the deceased if the deceased mempunyai pasport or akaun bank or insurans (original and copy);
5. Certificate Nikah or marriage document for application that dibuat by husband or wife the deceased;
6. Documents-documents lain that bby menyokong application.

1. Application Fee RM50.00
2. Fees extract Death Certificate RM5.00

1. NRD.LM21 – Form Application Kemasukan Nama/Correction Information Di In Register Birth Dan Death.
2. Statutory Declaration AM 80  – Surat Akuan Application Correction Fakta In Register Birth/Death 
   (obtain at the NRD counter) has been completed in full.
