Application to Correct Information Under Section 31(2) in the Death Certificate for Clerical Error (Sabah)

Application to membetulkan kesilapan ejaan that mana jika correction dibuat tidak akan mengubah fakta di in Death Certificate.

Application correction information section 31(2) may be made by the following individuals:

1. Biological mother or father the deceased;
2. Husband or wife the deceased;
3. Biological siblings the deceased;
4. Biological child or adopted child that sah to the deceased;

1. Form PSB 4 and PSB 4A that complete;
2. Death Certificate the deceased (original and copy );
3. Identity Card or identification document applicant (original and copy )
4. Certificate Nikah or marriage document for application that dibuat by husband or wife the deceased;
5. Documents-documents lain that bby menyokong application.

Fees registration application RM5.00;

Fees extract Birth Certificate RM10.00.
