Delayed Registration of Death (Sabah)

Applicantan registration death lambat bermaksud registration death dibuat pada hari ke-2 hingga hari ke-10 after death berlaku.

Application for death registration may be made by the following individuals:

1. Next of kin the deceased;
2. Individu that mengetahui tentang death.

1. Form B and Form N2 (extract) diisi with complete;>
2. Reports polis for death that berlaku di rumah (original and copy);
3. Pengesahan or perakuan death jika death berlaku di hospital (original and copy);
4. Identity Card or Permit Masuk or Pasport informant(original and copy);
5. Identity Card or identification document the deceased. Jika tiada/hilang laporan polis must be enclosed.

Processing fee RM5.00.
