Delayed Registration of Death (Sarawak)

Applicantan registration death lambat bermaksud registration death dibuat pada hari ke-2 hingga hari ke-7 after death berlaku.

Application for death registration may be made by the following individuals:

1. Next of kin the deceased;
2. Individu that mengetahui tentang death.

1. Form III and Form XI diisi with complete;
2. Pengesahan Ketua Masyarakat or Ketua Kaum the deceased;
3. Reports polis for death that berlaku di rumah (original and copy);
4. Medical Certificate of The Cause of Death (Form VIII);
5. After Post-Mortem (Form IX);
6. Identity Card or Permit Masuk or Pasport informant (original and copy );
7. Identity Card or identification document the deceased;
8. Reports polis jika Identity Card or identification document the deceased tidak diserahkan or tidak dijumpai to NRD.

Processing fee RM5.00.
