Extract of Death Certificate (Sabah)

Application to mendapatkan copy death register by applicant that berkelayakan with fees that designated. Application extract death certificate ini bby dibuat di NRD State and NRD Putrajaya sahaja.

Application for an extract of death certificate may be made by the following individuals:

1. Mother or father the deceased ;
2. Husband or wife the deceased ;
3. Biological child or adopted child that sah for pemilik Death Certificate;
4. Biological siblings pemilik Death Certificate;
5. Legal Firm;
6. Statutory Body;
7. Agensi kerajaan.

1. KATEGORI 1: INDIVIDUALS IN CATEGORIES A TO D
   • Form N2 that complete;
   • Identity Card or identification document applicant (original and copy);
   • Copy death certificate the deceased (if ada).Belum pernah berkahwin.
     
2. KATEGORI 2: INDIVIDUALS IN CATEGORY E
   • Form N2 that complete;
   • Official Letter Office Legal Firm to memohon search and extract in register;
   • Appointment letter from client (applicant);
   • Identity Card or identification document client (applicant) (copy);
   • Authorization letter to menguruskan application (if applicable).
     
3. KATEGORI 3: INDIVIDUALS IN CATEGORY F DAN G
   • Form N2 that complete;
   • Official Letter Statutory Body or Government Agency to memohon search or extract in register;
   • Authorization letter applicant to menguruskan application (if applicable).

Fees application RM5.00. Manakala application from Government Agency is dikecualikan fees.

1. FormN2 (obtain at the NRD counter) has been completed in full.
