Late Registration of Death (Peninsular)

Applicantan register lewat death is death that berlaku di Malaysia and diregisterkan after hari ke 7 (death that berlaku before 1 Disember 2011 sahaja). Registration death bby dibuat at any NRD office throughout Peninsular Malaysia.

The application must be submitted by dua (2) orang sponsors that merupakan next of kin the deceased or orang that mengetahui tentang death tersebut. Jika tiada next of kin, sponsors ialah pegawai polis that menjalankan siasatan.

1. Form Death Register NRD.LM02;
2. Statutory Declaration Form NRD.LM28;
3. Documents pengenalan diri the deceased;
4. Identity Card or identification document informant (original and copy);
5. Documents Death Certification NRD.LM09 or NRD.LM10 (jika death berlaku di hospital);
6. Death Register or Permit Kubur NRD.LM02 (Pin.1/11).

Application Fee: RM50.00

1. NRD.LM02 (Pin.1/11) – Form Death Register 
2. NRD.LM28 – Form Application Registration Lewat Death 
   (obtain at the NRD counter) has been completed in full.
