Late Registration of Death (Sabah)

Applicant register lewat death is death that berlaku di Sabah and diregisterkan after hari kesepuluh dari date death.

Application registration lewat death may be made by the following individuals:

1. Mother of the deceased;
2. Father of the deceased;
3. Child of the deceased;
4. Siblings the deceased;
5. Wife or husband the deceased;
6. Police officer that menjalankan siasatan jika tiada next of kin to the deceased.

1. Form B and Form N2 (Extract);
2. Documents pengenalan diri the deceased.;
3. Identity Card or identification document informant (original and copy);
4. Reports Polis for death that berlaku di rumah and pengesahan by pegawai perubatan jika death berlaku di hospital/klinik;
5. Keterangan Ketua Anak State or Ketua Kampung and Akuan Saksi.

Processing fee RM10.00.
