Late Registration of Death (Sarawak)

Applicant register lewat death is death that berlaku di Sarawak and Informant reports death after tujuh (7) hari berikutnya dari date death.

Application registration lewat death may be made by the following individuals:

1. Mother of the deceased;
2. Father of the deceased;
3. Child of the deceased;
4. Siblings the deceased;
5. Wife or husband the deceased;
6. Police officer that menjalankan siasatan jika tiada next of kin to the deceased.

1. Form III and Form XI;
2. Pengesahan Ketua Masyarakat or Ketua Kaum for Sarawak;
3. Form VIII (Medical Certificate of the Cause of Death);
4. Form IX (After Post-Mortem);
5. Identity Card or identification document informant (original and copy);
6. Documents Introduction Diri the deceased;
7. Reports Polis jika identity card or Certificate or Extract Register Birth Si mati tidak diserahkan.

Processing fee RM10.00.
