Ordinary Registration of Death (Peninsular)

1. ACTION BY DEATH VERIFICATION AGENCY (HOSPITAL, PDRM & DBKL)
   Receives information death from informant;
   • Verifies and issues Death Register / Burial Permit NRD.LM02 (Pin.1/11) and submits colour biru to informant death;
   • Sends copy Death Register / Burial Permit NRD.LM02 (Pin.1/11) colour kelabu to NRD that nearby in masa 7 working days and NRD akan merekodkan information death di in sistem I-NRD. Informant or next of kin the deceased bby collect death certificate di office NRD that telah designated.
     
2. DOCUMENTS TO BE SUBMITTED BY THE INFORMANT OR NEXT OF KIN WHEN PRESENT AT THE NRD OFFICE
   • Copy Death Register/Burial Permit NRD.LM02 (Pin.1/11) that colour biru;
   • Identity Card or identification document the deceased that original;
   • Identity Card or identification document informant; (original and copy);
   • Perakuan by Medical Practitioner mengenai sebab-sebab death that original [Form NRD.LM09 or NRD.LM10 (Post-mortem)] if death berlaku di hospital.
     
3. INDIVIDUALS ELIGIBLE TO COLLECT THE DEATH CERTIFICATE
   • Mother or father the deceased;
   • Husband or wife the deceased;
   • Children of the deceased;
   • Biological siblings the deceased
   • Informant to death that tercatat in Death Register/Burial Permit NRD.LM02 (Pin. 1/11).
     
4. FEES
   • No fee is charged.

1. ACTION BY NEXT OF KIN OR INFORMANT
   • Reports death to Death Verification Agency (Hospital or Polis or DBKL);
   • Receives copy Death Register/Burial Permit NRD.LM02 (Pin.1/11) colour biru from death verification agency;
   • Submits satu copy NRD.LM02 (Pin.1/11) to Penjaga Tanah Kubur to purpose burial; and
   • Present ke NRD nearby to menregister and mendapatkan Death Certificate.

   • Copy Death Register/Burial Permit NRD.LM02 (Pin.1/11) that colour biru;
   • Identity Card or identification document the deceased that original;
