Ordinary Registration of Death (Sarawak)

Applicantan registration death biasa bermaksud setiap death that berlaku di Sarawak must diregisterkan in period 24 jam dari waktu death.

Application for death registration may be made by the following individuals:

1. Next of kin to the deceased;
2. Individu that mengetahui tentang death.

1. Form III and Form XI diisi with complete;
2. Pengesahan Ketua Masyarakat or Ketua Kaum the deceased;
3. Reports polis for death that berlaku di rumah (original and copy);
4. Medical Certificate of The Cause of Death (Form VIII);
5. After Post-Mortem (Form IX);
6. Identity Card or Permit Masuk or Pasport informant (original and copy);
7. Identity Card or identification document the deceased. Jika tiada/hilang laporan polis must be enclosed.

Free of charge

1. Form III
2. Form XI 
   (obtain at the NRD counter) has been completed in full.
