Report of Death Abroad (Peninsular)

Setiap death penduduk Malaysia di abroad must be reported ke National Registration Department (NRD). Jabatan ini akan mengemaskini information death the deceased di in sistem NRD and satu (1) surat Perakuan Pengesahan Death akan dikeluarkan to applicant. Reports death abroad bby dibuat at any NRD office throughout Peninsular Malaysia.

Application laporan Death abroad may be made by the following individuals:

1. Husband or wife the deceased;
2. Child of the deceased that aged 18 tahun and ke atas;
3. Mother or father the deceased;
4. Biological siblings the deceased that aged 18 years and above;
5. Individu that mengetahui tentang death and dapat mengemukakan keterangan or bukti mengenai death.

1. Form Death Register NRD.LK01 (Kemaskini Death Abroad);
2. Identity Card or identification document the deceased that original;
3. Identity Card or identification document applicant (original and copy);
4. Proof of death documents:-
   • Death Certificate that dikeluarkan by negara dimana death berlaku (documents original and satu (1) copy documents original);
   • Surat Pengesahan Death from hospital di mana death berlaku (original and copy ); and
   • Surat Pengesahan Death from Office Kedutaan Malaysia di abroad di mana death berlaku (original and copy ).
