Report of Presumed Death (Sabah)

Application to mengemaskini information individu that telah dianggap mati through court order hanya diuruskan by NRD Ibu Office Putrajaya sahaja with pengeluaran surat makluman for pengemaskinian status individu that telah dianggap mati. Walaubagaimanapun, informant bby juga mengemukakan application di NRD Ibu State to dipanjangkan ke NRD Ibu Office Putrajaya.

Application for a presumed death report may be made by the following individuals:

1. Mother or father pemilik Court Order;
2. Husband or wife pemilik Court Order;
3. Biological child or adopted child that sah for pemilik Court Order;
4. Biological siblings pemilik Court Order;
5. Legal Firm;

1. Form NRD.LM02;
2. Perintah Anggapan Death from High Court or Syariah High Court;
3. Surat application from Legal Firm (if the applicant is Legal Firm);
4. Identity Card or identification document applicant (original and copy);
5. Identity Card client (if the applicant is Legal Firm) (copy);
6. Kad pengenalan the deceased.

Free of charge

1. NRD.LM02 – Form Death Register 
   (obtain at the NRD counter) has been completed in full.
