Search / Extract of Death Register (Peninsular)

Application to mendapatkan search / extract death register by applicant that berkelayakan with fees that designated.

Application search / extract register Death bby dibuat by kategori individu seperti berikut:

1. Mother or father (termasuk mother or father angkat or mother or father tiri)
2. Husband or wife that sah.
3. Anak (termasuk adopted child or stepchild seibu/sebapa).
4. Siblings (termasuk siblings kandung/siblings angkat/siblings seibu/sebapa).
5. Datuk/nenek dari ibu/bapa kandung.
6. Cucu kandung.
7. Legal guardian (disertakan surat kebenaran penjagaan child dari JKM (Form 13) / Perintah Mahkahmah.
8. Legal Firm (mewakili agensi swasta/GLC/Institusi Kewangan/lain-lain individu that tidak berkaitan with pemilik documents).
9. Statutory Body (agensi must memohon through lawyer/tetuan masing-masing).
10. Government Agency.

Applicant is required to be present di NRD nearby seluruh Peninsular Malaysia.

A. INDIVIDUALS IN CATEGORIES 1 TO 7

• Form NRD.LM13 (death di Peninsular)/ Form N2 (death di Sabah)/ Form XI (death di Sarawak) which has been completed in full (original).
• Identity Card or identification document applicant (original and copy).

B. INDIVIDUALS IN CATEGORY 8

• Form NRD.LM13 (death di Peninsular)/ Form N2 (death di Sabah)/ Form XI (death di Sarawak) which has been completed in full (original).
• Official Letter dari Office Legal Firm to memohon search / extract in death register (original).
• Appointment letter from client (original).
• Identity Card or identification document lawyer (original and copy).
• Identity Card or identification document client (copy that telah disahkan by Office Legal Firm).
• Authorization letter to menguruskan application and identification document applicant (original and copy – if applicable).
• Legal Practice Certificate that sah (original/copy that disahkan by lawyer lain.

C. INDIVIDUALS IN CATEGORY 9 DAN 10

• Form NRD.LM13 (death di Peninsular)/ Form N2 (death di Sabah)/ Form XI (death di Sarawak) which has been completed in full (original).
• Official Letter dari Statutory Body or Government Agency to memohon search / extract in death register (original).
• Authorization letter to menguruskan application and identification document applicant (original and copy – if applicable).

Application Fee (Keputusan Search Death Register) – RM10.00

Extract Death Register – RM5.00

2. Form N2 (death di Sabah)
3. Form XI (death di Sarawak)
