Application for Extracts of Identity Card Register and Address Register

Regulation 12(1) and (2) of the National Registration Regulations 1990 (Amendment 2001) provides that the register shall not be opened for public inspection. No public officer, unless duly authorised by the Director-General, or a police officer in the exercise of powers under Chapter XIII of the Criminal Procedure Code, may inspect the register. However, extracts from the register may be issued to public officers at the discretion of the Director-General, or to members of the public upon payment of the fee prescribed by the Regulations.

1. Extract of Identity Card Register (JPN.KP12)
   • The applicant must already possess an identity card.
   • The applicant or beneficiary must be present at any NRD office.
   • The nearest beneficiaries are as follows:
     – Mother or father of the owner
     – Adoptive mother or father of the owner
     – Child of the owner
     – Husband/wife
     – Grandfather or grandmother
     – Siblings
     – Legal guardian
   • Application Form JPN.KP02 (obtainable at the NRD counter) must be completed in full.
2. Extract of Address Register (JPN.KP13)
   • The applicant or representative (government agency/enforcement agency/statutory body/legal firm) must submit complete information to the NRD office through the counter, official departmental email or letter for prior checking.
   • The application letter must be submitted by an applicant with a valid official letterhead. Applicant documents must be original documents and copies.

1. Extract of Identity Card Register (JPN.KP12)
   • The applicant/beneficiary/representative must be present at an NRD counter or post the application to NRD Headquarters Putrajaya or the State NRD Headquarters.
2. Extract of Address Register (JPN.KP13)
   • The applicant or representative (government agency/enforcement agency/statutory body/legal firm) must submit complete information to the NRD office through the counter, official departmental email or letter for prior checking.
   • The application letter must be submitted by an applicant with a valid official letterhead.
   • Applications made by email must be sent accordingly.

1. Extract of Identity Card Register (JPN.KP12)
   • Documents submitted by the owner are as follows:
     – Applicant’s identity card or a copy; or
     – Entry Permit; or
     – Other documents such as Birth Certificate, Death Certificate, Driving Licence, Passport, bank book or other documents that record the identity card number.
   • Documents submitted by the nearest beneficiary are as follows:
     – Applicant’s identity card; or
     – Birth Certificate of the applicant and owner; or
     – Death Certificate; or
     – Marriage Certificate; or
     – Other documents proving the relationship with the owner.
2. Extract of Address Register (JPN.KP13)
   • Documents submitted by the representative/government agency/enforcement agency/statutory body/legal firm:
     – Letter from the legal firm on behalf of the third party/representative.
     – Application letter with official letterhead from the government agency/enforcement agency/statutory body/legal firm.
