Only original forms are accepted. Forms can be obtained at all NRD offices.
Fee
No payment imposed.
Validity
Six (6) months from the date of approval.
General Information
Marriage for someone who is 18 years old but not yet 21 years old.
Application Requirements
Applicants must comply with the basic conditions of the civil marriage registration application.
The application must receive written approval from one of the parties below:
Father or mother; or
His mother if a person is illegitimate; or
Adoptive mother or father if a person is an adopted child; or
Guardian for a person if both his parents, whether biological or adoptive parents, have died before reaching the age of 21; or
Apart from the situation above, permission from the High Court of Malaysia is required
The applicant and the party that giving consent must appear before the marriage registrar or Malaysian representative if they are outside the country.

MyKad/ MyPR/ MyKAS/
Birth certificate (if below 21 years old)
One (1) passport size color photo with a blue or white background for each applicant.
Non-Citizen
A valid passport and visa containing information on the date of arrival in Malaysia (original and copy)
Confirmation letter of marital status from the government of the applicant's country (original and copy)
Birth Certificate (original and copy)
One (1) passport size color photo with a blue or white background for each applicant.
Parent / Guardian giving consent
MyKad/MyPR/MyKA (original and copy)
Valid Passport / Identification document (original and copy)
If require:
Divorce Certificate (original and copy)
Death Certificate (original and copy)
Change name certificate (original and copy)
ID verification / Citizenship (original and copy)
