Form JPN.KC01F – Marriage Ceremony Application Form According to Customary Religious Ceremonies or Custom.
Only original forms are accepted. Forms can be obtained at all NRD offices.

Validity
6 months from the date the application is submitted.
General Information
The Assistant Registrar of Marriages must be legally appointed by the Registrar General of Marriages
One of the spouses must adhere to the religion, customs and traditions practiced by the Religious Institution/Association.

Application Documents
Only Malaysian citizens may apply for civil marriage at the Assistant Registrar of Marriages office.
Couples need to come to NRD to verified marital status information.
Both applicants are required to make a Statutory Declaration using Form JPN.KC01F before submitting the application at the NRD counter.
Only complete applications will be accepted and processed.
Required Documents
MyKad (Original)
One (1) color photograph of passport size with a blue or white background for each applicant.
If require:
Divorce Certificate (original and copy)
Death Certificate (original and copy)
Specific Documents for Police/Army Applicant
Police officer
Confirmation Letter of Permission to Marry (original and copy) or;
Appendix A (HRMIS) complete signed by superior officer.
Military officer
Confirmation Letter of Permission to Marry from the place of duty (original and copy)
