Marriage Licence for a Prohibited Relationship

Original Form JPN.KC01A – Marriage license application form for parties with prohibited kinship ties.
Only original forms are accepted. Forms can be obtained at all NRD offices.
Fee
RM2.00 if the application approved.
Validity
One (1) month from the date of approval.
General Information
Marriage for parties who are related to each other is prohibited referring to section 11 of the Marriage and Divorce Law Reform Act 1976 (Act 164).
This application is not necessary for the marriage of a Hindu couple between a sister's children or with a younger brother or maternal uncle. In addition to the Hindu religion, it is necessary to obtain special permission for this situation.
The approval of this application is subject to the approval of the Chief Minister/ Divisional Director/ State Director
Birth certificate holder with undetermined citizenship status or adoption registration with non-citizen status issued by the Malaysian government are not allowed to apply to register a marriage with any license.

Only applications with complete documents will be processed. Applicants must submit any additional supporting documents requested by the NRD officer until the officer is satisfied with all the documents.

Marriage Licence for Exemption from Notice

Original Form JPN.KC01C – Marriage License Application For Exemption From Displaying a Marriage Notification.

RM100 if the application approved.
Validity
One (1) month from the date of approval.
General Description
Application from a couple who wants to waive the period of displaying the marriage notification notice for certain reasons.
The approval of this application is subject to the approval of the Chief Minister/ Divisional Director/ State Director.
Birth certificate holder with undetermined citizenship status or adoption registration with non-citizen status issued by the Malaysian government are not allowed to apply to register a marriage with any special permission.

One (1) passport size color photo with a blue or white background for each applicant.
Non-Citizen

A valid passport and visa containing information on the date of arrival in Malaysia (original and copy)
Confirmation letter of marital status from the government of the applicant's country (original and copy)
Birth Certificate (original and copy)
One (1) passport size color photo with a blue or white background for each applicant.

Divorce Certificate (original and copy)
Death Certificate (original and copy)
Change name certificate (original and copy)
ID verification / Citizenship (original and copy)
Specific Documents for Police/ Army Applicant
Police officer
Confirmation Letter of Permission to Marry (original and copy) or;
Appendix A (HRMIS) complete signed by superior officer.
Military officer
Confirmation Letter of Permission to Marry from the place of duty.

Marriage Licence for a Woman Aged 16 to Below 18

This application is a marriage license for a woman who is 16 years old but not yet 18 years old

The applicant is the mother or father/guardian of a girl under the age category of 16 years and not yet 18 years old.
The written form and declaration must be filled out and signed in front of the marriage registrar.

Written Declaration form of marriage by Mother/ Father/ Adoptive Mother/ Adoptive Father/ Guardian.
Identity document for the person giving consent (original and copy)
Pregnant mother's health report (if relevant)
Document the financial resources of male and female couples:
If working with an employer – Income slip/ financial statement/ bank statement/ Savings statement
If self-employed – Confirmation of Village Head / Chief / Tribe Head / Carrier/ Harvester.
Not working – Affidavit explaining source of income.

Marriage Solemnisation Outside the Registrar’s Office

Form
ORIGINAL Form JPN.KC01E – Application form for a license to solemnize a marriage in a place other than the registrar's office that has been completely filled out.

RM500.00 if the application approved.
Validity
One (1) month from the date of approval
General Information
Special permission for Marriages solemnization outside Registrar office.
The place of solemnization ceremony that are allowed must be a suitable and safe place for the ceremony.
The time of the wedding ceremony is subject to the schedule of the registrar on duty. The registrar will solemnize the marriage within 2 hours only for one ceremony.
The approval of this application is subject to the approval of the Chief Minister/ Divisional Director/ State Director.
Application Requirements
Applicants must comply with the basic conditions of the civil marriage registration application.
Marriage applications under this license can only be accepted at the Putrajaya NRD Headquarters or the NRD State offices depending on the venue chosen.
The couple must be present together with all the forms as well as a written declaration that must be filled out and signed in front of the Marriage Registrar.
The approval of the application depends on the capacity of the officer and the purpose of the interview conducted outside the office.
